We would like to know how we can improve our service to you and how you perceive our surgery and staff.
To help us with this, we have set up a patient representation group so that you can have your say. We welcome patients from all age groups, social and cultural backgrounds to join. We meet on a regular basis and are always looking for new members. If you want to become involved in local healthcare decision making, please complete either of the forms below.
